Insurance Enrollment Process

We are delighted that you would like to enroll your child at MAPS. If you have not already visited our campus and seen our center –based programs, then you may want to request a tour. If you would like to get started on enrollment, these steps will clarify the process.

Step One: Let Us Know

Contact our Intake Department and let them know that you are interested in having your child receive services at MAPS. You can email our Intake Department at shelbyl@mapsautism.com or call us at (916) 357-5837 ext. 102.

We have different availability depending on the service requested; in-home ABA, on-site ABA, group social skills (playgroups), in-home mentor program, etc. To be placed on the assessment interest list, the parents must complete the Pre-Enrollment Packet. In addition to the Pre-Enrollment packet, we need:

  • Copy of all insurance cards, primary and secondary (front and back)
  • You can take a picture of your insurance cards and email them to maureenc@mapsautism.com

Once we have this information we will contact your insurance provider to verify coverage of services. We will notify you once your eligibility is confirmed and place your child on the waiting list. Please let us know if there is any change in your child’s availability.

Step Two: Complete the Client Enrollment Packet for Assessment

For us to place your child in the program we will need to do an initial assessment.  The assessment provides us with relevant information regarding your child’s basic skills, gifts, challenges and social development. On our website, you will find an online Client Enrollment Packet. Complete and submit the packet via our website. If you need help completing the packet, you can make an appointment to meet with our Intake Specialist for assistance.

Additional documents required:

  • Copy of Immunization Record
  • Child’s diagnosis report

Before we can begin your child’s assessment we must receive all the above documents.

 ***We will request authorization for assessment from your insurance company. Authorizations take 2-10 business days to receive. Once received, we will assign your child to an assessor who will contact you to schedule assessment meetings.***

Step Three: Scheduling Initial Parent Meeting

Timeline to Complete Assessment: 30 business days

Once we verify all documents are returned and completed, we will contact you to schedule the Initial Parent Meeting.

Step Four: Initial Parent Meeting

We will schedule a 1-2-hour meeting to gather your child’s skills, deficits, developmental levels and preferences using specific assessment tools. We will go over the Safety Checklist and Parent Roles and Responsibilities and answer any questions.

Step Five: Natural Play Environment Observations

A Natural Play Environment is any place that your child typically plays and interacts with peers.  A suitable environment includes; school, lunch/recess, places like gymnastics, karate, or even the child’s neighborhood park are also appropriate.

Step Six:  MAPS Observation

This step of the assessment is performed on our campus.  We ask that you bring your child at a prearranged time. Parent participation is not needed so plan for a drop-off and pick up. Please feel free to spend some time at the beginning; making sure that your child is well accommodated.

Step Seven:  Assessment Report

We will submit a complete assessment report to your insurance company and request an authorization for service. When we receive authorization, we will contact you to with an Initial Offer of Services.

Step Eight:  New Parent Orientation

We will schedule a parent orientation meeting prior to your child’s program beginning. We will review the assessment report and the parent handbook as well as your child’s upcoming program.
